We are currently searching for a Pregnancy Support Case Coordinator in our Harrisburg Outpatient clinic.

Persons who would be the best fit for this position would have a history of working with those with substance use disorders, maintain a compassionate yet boundaried approach to a high needs population, and be organized and detail orientated.   EOE

In this position you will:

  • Work with women who are pregnant or recently postpartum and a history of substance use disorders
  • Collaboration of systems and services is an important factor for increasing positive outcomes for individuals who are in treatment
  • Work directly with individuals with substance use disorders and provide support to bridge connections between treatment providers, health care professionals, insurance agencies, elicit natural supports and other services that may be beneficial to recovery
  • Meet with individuals on a one on one basis to support them in learning skills and techniques to better support their infants and small children
  • Collect and monitor program specific data
  • Complete level of care assessments for new clients
  • Teach weekly psycho educational groups
  • Communicate with clients in accordance with motivational interviewing and other designated evidence-based procedures
  • Refer clients to other services as needed
  • Assist clients in accessing external resources, including treatment, housing, medical care and other criminogenic and/or psychosocial needs
  • Must be willing to cover for the childcare worker when needed
  • Research available resources in the community for clients and refer when appropriate
  • Will be based out of the Harrisburg office but will also cover Cumberland, Dauphin, Franklin/Fulton and Perry County, must be able to travel to these additional locations when necessary

$16.35 per hour
High School Diploma or Bachelor's Degree
Monday - Thursday 8:00am-6:00pm
